Use these handy LVL Treatment Record Cards to record important information that can be noted during LVL lash lifts, such as shield sizing and timings. Ensure all details are recorded during the treatment process and for best practice, we recommend both clients and lash artists sign and date the cards.
For best practice
Write in ink
Always use ink to complete the Treatment Card and never delete any mistakes. If you make a mistake, cross it out and write the correction after the error. Alternatively, you can start a fresh Treatment Card.
Complete fully
Always do a final check to ensure all elements of the LVL Treatment Record Card has been completed in full.
