These professional cards replace our old Client Record Cards and Lash Extensions Treatment Record Cards so you can store all your client’s essential treatment information in one place. 

There is space to record client information and their full medical history. They provide documented evidence that your client has given you authorisation to carry out a lash extensions treatment and confirms that they understand the nature and method of the treatment.  

You can also use these handy Lash Extensions Treatment Cards to record important information that needs to be noted during treatments, such as products used and lash lengths, curls and weights. There’s even a lash map to enable you to draw out the look during the consultation and keep for reference for the next appointment.  

Each card has space to store details for five treatments. They are suitable for Extend, Global Volume, SVS and Hybrid treatments, as well as maintenance and removal appointments. 

Ensure all details are recorded during the treatment process and for best practice, we recommend both clients and lash artists sign and date the cards.
